I/We understand that the WCJFL, INC. has NO INSURANCE on any of its players/cheerleaders.
I/We understand that our child cannot play football in (2) different leagues at the same time during a season.
I/We give our consent for said child above, to enroll in WCJFL INC. We hereby waive and relinquish any/all rights of legal action or claims against said League or the Officers, members, Coaches and Cheer Advisors thereof, which might hereafter exist in the event of athletic injuries to said child.
I/We give permission to WCJFL INC., to videotape, photograph, and/or televise said child.
I/We understand that it will be our responsibility to furnish transportation for our child to and from practices/games. I also understand that it is our responsibility to assist the League in fundraising activities such as Concession Stand. I understand that lack of participation in such activities will result in a one game suspension for my child.
I/We understand that there is a $75.00 ($85.00 after May 31)
sign-up Fee at the time of registration for all Football Players and $65.00 ($70.00 after May 31)
sign-up fee for each additional sibling, living in the same household. Flag Football players will be $45.00 ($50.00 after May 31)
and each additional Flag sibling, living in the same household, $35.00 ($40.00 after May 31)
. Cheerleaders will be $50.00 ($55.00 after May 31)
and each additional Cheer sibling, living in the same household, will be $35.00 ($40.00 after May 31)
. If a Player/Cheerleader reside in the same household, the Player fee takes precedence, with the exception of Flag, Cheer fee will take precedence. CASH, CREDIT CARD OR MONEY ORDERS ONLY, NO CHECKS. ALL FEES ARE NON-REFUNDABLE. These fees are for League function only (i.e., uniforms, field maintenance, electric, etc) and do not include any Team Fundraising.
I/We agree to return all equipment and uniforms to the Coach or League no later than (30) days
after the last game played. If equipment is not returned I/We will be subject to prosecution.
I/We Understand That A Physical Evaluation
(please print for physician completion and turn in FIRST night of practice). Must Be Completed for Players/Cheer And Returned Prior To Participation. Physicals MUST be completed annually.
